How to Obtain an Official License Verification

To officially verify your Ohio license to another entity, you must submit a written request that includes the following information:

  1. Your name;
  2. Your current credential mail address;
  3. Your daytime telephone number;
  4. Your Ohio license number and/or the last four digits ONLY of your Social Security Number;
  5. Your date of birth;
  6. Your current business address and name of the business (indicate if this is your employer or your practice site);
  7. The mailing address of the entity requiring the verification

You may also send the Board a copy of the other entity's verification form provided that it includes your name, Ohio license number and daytime telephone number. There is no fee for license verification to another entity.

Verifications are processed within 7-10 business days of receipt of the request. Requests received via e-mail will receive notification of when the verification has been processed.
