name, address and employment changes are required to be made
in writing within thirty days of the change and may be
emailed to board@slpaud.state.oh.us,
faxed to 614/995-2286 or mailed to the Ohio Board
of Speech-Language Pathology and Audiology, 77 South High Street,
16th Floor Columbus, Ohio 43215. Note: Unreported address
changes may delay or prevent receipt of important information
concerning license renewal or application status.
