Business Filings are required reflecting every odd calendar year; DUE DATE: March 1st of every even year.

The next filing will be posted following the end of calendar year 2009 and must be submitted on or before March 1, 2010.

  • Facsimile (614) 995-2286. You may fax the list of licensee names on the required board approved form in advance HOWEVER, the original notarized page must be mailed.
